Toggle option per appointment type to allow redirecting of public intake link to a "sorry we are full" page instead of the sign up form

There are some pharmacies with many thousands of people on the waitlist for certain appointment types - more than we can get to in a reasonable time. We would like to redirect people to a page where we can post a "Sorry, but we are full" type of message. Turning off public intake without this only causes more calls to the pharmacy asking about signing up.

agreed with James, the list grows everyday and patients calling or coming in that they have been on the waitlist for a long time and haven't heard back. It's because we have 3000 people on the waitlist and will never be able to help them all...
